Admission Procedure

PROSPECTUS & APPLICATION FORM

• Application will be invited for admission into 2(Two) yrs Health Worker (F)/ANM training course from intending candidates through publication of advertisement in daily News Paper.
• Prospectus and application form will be available from School by depositing Rs.200/- (Rupees one hundred) by cash and Rs.250/- by DD in favor of “Dox-Orland School of Nursing”
• payable at Keonjhar from 15-08-2016 to 31-08-2016 by 10A.M to 5 P.M in working days.
• Last date of submission of application is 07-09-2016 by Hand/ By  Post before 5 P.M. to the Principal Dox-Orland School of Nursing.
• The office should not be responsible for delay or loss in transit.​​

DISTRIBUTION OF SEATS

• Out of the total number of seats 50% will be taken from the central counseling of the Govt. of Odisha.
• S.C/S.T & other categories of candidates will be taken as per the Govt. guidelines .
• The remaining seats will be filled up by the Management Quota.
• The seats remaining vacant of the central counseling seats will be filled up by Management quota seats. 

EDUCATIONAL QUALIFICATION & SELECTION CRITERIA

•  +2 Science with 55% of marks.(50% for ST & SC student)
•  +2  Arts and Commerce with 60% of marks.
• +2 Science students are preferred first.
•  Age limit : Only the female candidates  completed 17 years and below 35 years as on 31-07-2016,  are eligible to applay.
• Must have passed Odia upto M.E. standard.
• Must be a permanent resident of Odisha.
• The selection of the candidates for the management quota seats will be on mark basis , Formal written test and personality test.
• The results of the selected candidates will be display in the school notice board and candidates will be intimated over telephone or mobile.​​​

​DOCUMENTS TO BE SUBMITED ALONG WITH THE APPLICATION FORM 

• Self Attested copy of H.S.C. & C.H.S.E/+2 science/equivalent pass certificate.
• Self Attested copies of mark sheet of H.S.C. & C.H.S.E./+2 Science/Arts/Commerce or equivalent pass certificate .
• Self attested true copy of Conduct & Character certificate.
• Recent 4 passport size and 2 stamp size photographs . 2 passport size photographs should be duly attested in the front side by a Gazetted Officer.
• Self attested true copy of Caste certificate.
• Any other certificate(NCC/SCOUT/NSS/Green Card).
• Self attested copy of  Odia pass certificate upto M.E. standard.

​N.B:-

Cast certificate and residential certificate are to be produced only from District Magistrate/ Additional District Magistrate/ Sub-Divisional Officer/ Tahasildar/ Additional Tahasildar.

DOCUMENTS REQUIRED AT THE TIME OF ADMISSION

• Original  H.S.C. & +2 pass certificate.
• Original  SLC / CLC.
• Original Mark sheet.
• Original Caste & Residential certificate.
• Original Conduct Certificate.
• Original H.S.C or other authentic proof of passing Odia upto M.E standard.
• Original P.H certificate/ Ex-Serviceman Certificate / G.C.
• Affidavit from a notary in a prescribed format  at the time of admission as per the annexure of Govt. Prospectus. .
• Medical fitness certificate.

N.B:-

1. The candidate must not  be mentally ill and must be physically fit for the nursing work.

2. Undertaking for extension of time for submission of original documents will not be entertained except original SLC/CLC during counseling .

But all original certificates have to be produced during joining positively failing which for admission will be canceled and the amount paid by the candidate towards the admission fee will be forfeited .

3. If any candidate found to have produced any false information / hide any fact, her candidature will be cancelled and deposited money will be forfeited.
